A Crisis Hidden in Plain Sight

Walk through any market in Raipur — Pandri, Shastri Bazaar, Telibandha — and you will find gutka, khaini, and tambakoo sold openly at every paan shop. Chhattisgarh has one of the highest rates of tobacco consumption in India, and the price is devastating: oral cancer accounts for a disproportionately large share of all cancer cases in our state.

The tragedy is not just in the numbers. It is in the fact that oral cancer is one of the most preventable and detectable cancers — and yet, most patients in Raipur and surrounding districts like Durg, Bilaspur, and Korba reach a doctor only when the disease is already in an advanced stage.

As a Radiation Oncologist at the American Oncology Institute (AOI-BCR), Jora, Raipur, I see this pattern every week. This blog is my attempt to change it — one family at a time.

What Is Oral Cancer?

Oral cancer refers to cancer occurring in any part of the mouth — the lips, tongue, inner cheeks, gums, floor of the mouth, and the roof of the mouth (palate). It can also occur in the oropharynx, which includes the back of the throat, the base of the tongue, and the tonsils.

In Chhattisgarh, the most common sites are the tongue, cheek lining, and floor of the mouth — directly corresponding to where people place tobacco products.

Why Chhattisgarh Is at High Risk

Chhattisgarh's oral cancer burden is driven by several deeply embedded habits:

  • Gutka and khaini — areca nut combined with tobacco, often held in the cheek for long periods
  • Tambakoo with lime (chuna) — applied directly to gums and inner cheeks
  • Beedi smoking — common in rural and semi-urban areas including parts of Raipur district
  • Pan masala — even without tobacco, areca nut alone is a Group 1 carcinogen (WHO classification)
  • Alcohol consumption — significantly multiplies oral cancer risk when combined with tobacco

These are not occasional habits. For many families across Chhattisgarh, these are daily routines passed from one generation to the next — which is why early education within the family is so critical.

Early Warning Signs — Know These Before It Is Too Late

Oral cancer in its early stages is almost always visible — which makes early detection entirely possible with awareness. Watch for:

  • A mouth ulcer or sore that does not heal within 2–3 weeks
  • A white patch (leukoplakia) or red patch (erythroplakia) anywhere inside the mouth
  • A lump, thickening, or rough spot on the lips, gums, or inside the mouth
  • Difficulty chewing, swallowing, or moving the jaw or tongue
  • Persistent numbness or pain in the mouth, jaw, or neck
  • A swelling or lump in the neck that does not go away
  • Unexplained bleeding from the mouth
  • A feeling that something is caught in the throat that won't clear

Submucous Fibrosis — The Silent Precursor

One condition specific to gutka and areca nut users that is extremely prevalent in Chhattisgarh is Oral Submucous Fibrosis (OSMF). It begins as a stiffening and whitening of the inner cheeks, making it progressively harder to open the mouth fully.

OSMF is a pre-cancerous condition. Left untreated, it can progress to oral cancer. The good news: if caught early, it is manageable — and stopping tobacco use at this stage can halt its progression.
